Self referral

There are a number of services locally that you are able to make self referrals to.

 

Specialist referral

If your doctor refers you to a specialist for further care, you will be asked to book your appointment using the NHS e-Referral Service.

You will receive a letter from us within 2 weeks, which will include:

Instructions on how to book your appointment

  • Your booking reference number

  • A password, if required

Please follow the steps in the letter to book your appointment as soon as possible.

GP referrals

If you are concerned about any symptoms and would like to request a GP referral, please contact us using our GP appointent request form.

  • Select ‘I have a health problem’ from the available options.

  • Provide as much detail as possible about your symptoms or concerns.

Tracking a referral

The easiest way to track an existing referral is to contact the service you’ve been referred to directly.

You can find the service’s contact details on the referral letter we sent you, along with your booking reference number.

You can also track and manage some hospital referrals through the NHS App, depending on the service and your local hospital trust.

If your referral was made through the NHS e-Referral Service (e-RS), you may be able to:

  • View the status of your referral

  • Book, change, or cancel your first hospital appointment

  • See appointment details once confirmed
